Step One

Family Intake

Collects your roster and contacts so leaders can plan events and food. About 5 minutes.

  • Scout's name & additional scouts in the family
  • Parent / guardian names, phones, emails
  • Adults authorized to take your Scout to and from activities
  • Everyone attending events (headcount & food planning)
  • Medical flag so leaders know who to watch for

Step Two

Health Forms

Complete the official BSA Annual Health & Medical Record for each person attending. The form opens in its own window so it fills and scrolls smoothly.

One form per person. A parent attending with a scout fills it twice — once for the scout, once for themselves. Use the same Family / Household Name on each, and tap "Submit Another Response" after each one to start the next family member.

Family Talent Survey

Tell us how your family can pitch in. Pack 38 runs on volunteers — your skills, time, and connections help make our events happen.

Step Four

Pack Dues

Pay annual dues through our Zeffy campaign. Zeffy charges the pack no fees — 100% goes to Pack 38.
